Job summary

SCAN Health Plan is hiring a remote Care Coordinator to support members in aging in place. You will assess needs, coordinate benefits, and navigate the healthcare system using motivational interviewing and strong communication.

The role requires bilingual English and Spanish or other languages, 1+ year in healthcare or customer service, and confidentiality. Competitive pay, bonus program, generous PTO, and 401(k) are offered.

Qualifications

• High School Diploma required; some college preferred.
• Bilingual English and Spanish or Vietnamese, or Cantonese or Mandarin; test administered.
• 1+ years healthcare or customer service experience.
• Basic technical skills and good written and verbal communication.
• Ability to protect confidentiality and work in a compliant environment.
• Understanding CMS and DHCS regulations and local senior resources.

Responsibilities

• Assess and screen members to determine appropriate interventions and referrals.
• Triages and serves as contact point for members, providers, and vendors.
• Coordinate care using benefits and community resources; resolve access issues.
• Document assessments, care plans, and notes in multiple systems timely.
• Collaborate with county offices and government agencies as needed.
• Maintain professionalism and empathy in all interactions.
