Job summary

SCAN Health Plan in the United States is seeking a bilingual care planner to provide long-term care management to frail, economically disadvantaged older adults and caregivers. You will conduct in-home and telephonic assessments, develop care plans, and coordinate services.

The role requires travel in Los Angeles County, strong organizational and communication skills, and the ability to document accurately.

Qualifications

• Bachelor's degree in social work, psychology, gerontology, or related field.
• MSW preferred.
• Bilingual in English/Spanish; test may be administered.
• 1–2 years experience with the elderly; hospital/home health setting preferred.
• Ability to travel within Los Angeles County catchment area.
• Strong organizational, verbal and written communication skills.
• Experience in case management preferred.

Responsibilities

• Provide long-term care management to frail older adults and caregivers.
• Conduct in-home and telephonic assessments, care planning and client monitoring.
• Coordinate linkages with community services and resources.
• Maintain accurate documentation and timely case records.
• Ensure timely implementation of services within required timeframes.
• Travel within Los Angeles County as required.
• Support supervision and training of fellow staff as needed.
