Job summary

SCAN Group in Los Angeles is seeking a bilingual Care Planner to deliver long-term care management for older adults and caregivers, including home assessments, care planning, and coordination with community services.

The role requires a Bachelor's degree in social work, psychology, or gerontology (MSW preferred), and 1–2 years with the elderly, plus 50%+ travel within Los Angeles County. Remote/field-based work mode offered, with benefits and professional development.

Qualifications

• Bachelor's degree in Social Work, Psychology, Gerontology or related field; MSW preferred.
• Bilingual English/Spanish; proficiency tested.
• 1-2 years experience working with the elderly.
• 1+ year experience in hospital/home health focusing on older adults.

Responsibilities

• Provide long-term care management to frail and disadvantaged older adults and caregivers.
• Conduct in home and telephonic assessments, care planning, and client monitoring.
• Coordinate linkages with community-based services and resources.
• Maintain legal and clinical written records for each client.
