Job summary

Alignment Health is seeking a bilingual Spanish care coordinator for its remote Care Anywhere team. The role focuses on outbound member outreach after hospital or SNF discharges, coordinating timely follow-up care, and documenting activities across systems.

The position requires comfort with high-volume calling, adherence to outreach protocols, and strong organization. A minimum 1 year in health care and TOC experience are preferred, with a High School Diploma or GED and Medical Assistant

Qualifications

Must have 1+ year experience in Health Care (Health Plan, Medical office, IPA, MSO).
Experience conducting high-volume telephonic outreach to patients or members.
Experience with transitions of care (TOC), post-discharge outreach, hospital or SNF discharge follow-up.
Completion of an accredited Medical Assistant program or equivalent training.

Responsibilities

Conduct high-volume outbound telephonic outreach to members following hospital or SNF discharges.
Assist with scheduling timely follow-up appointments with Care Anywhere providers.
Document outcomes, referrals, authorizations and follow-ups in required systems.
Create cases and tasks in the Case Management module for discharges.
Coordinate referrals, authorizations, referrals, scheduling, and other care needs.
Maintain confidentiality and work with clinical teams to manage member needs.
