Job summary

UCB Inc. is seeking a compassionate, bilingual Onward Care Coordinator for the Rare Disease Patient Support team.

This remote role aligns with West Coast PST hours and supports Spanish- and English-speaking patients and caregivers throughout their treatment journeys. As a primary contact, you will deliver individualized, high-touch assistance focused on access, affordability, administration, and adherence, coordinating with Rare Case Managers and access specialists.

Qualifications

• Bachelor's degree required.
• 6+ years of patient-facing clinical experience; or 4+ years plus 2+ years industry years as a Registered Nurse, Doctor of Pharmacy, or similarly qualified clinical professional.
• Professional fluency in Spanish and English, including speaking, reading, and writing.
• Willing and able to work remotely during West Coast business PST hours.

Responsibilities

• Serve as the primary contact for Spanish- and English-speaking patients and caregivers.
• Deliver personalized support using empathy, active listening, and behavioral science skills.
• Explain insurance coverage, access pathways, and available affordability support options.
• Provide non-clinical education and refer medical questions to healthcare providers.
• Help patients prepare for productive conversations with their healthcare providers.
• Follow privacy, safety, adverse-event reporting, and program operating requirements.
