Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a Virtual Care Coordinator to support OhioRISE efforts across Region H. The role coordinates care for youth and families, leads teams at home, school, and community settings, and ensures voice and choice throughout services.

Responsibilities include facilitating team meetings, coordinating services with partners, maintaining documentation, and complying with HIPAA. Travel to service sites and virtual trainings are required.

Qualifications

• Experience providing services to children and families in behavioral health or related public sector.
• Two years in a coordinated supportive services or care coordination role preferred.
• Experience with autism spectrum disorders and developmental disabilities preferred.

Responsibilities

• Lead the team in identifying needs, goals, and action steps for youth and families.
• Develop, lead, and facilitate team meetings for the young person and identified team.
• Provide care coordination within the family’s home, community, or preferred location.
• Collaborate with social service providers and families to ensure best outcomes.
• Maintain documentation to access and maintain OhioRISE benefits and HIPAA compliance.
• Attend trainings and travel as required; operate in an internet-based office.
• Bill Medicaid without restrictions and ensure confidentiality.
