Job summary

Humana is seeking a Field Care Manager, Behavioral Health 2 to assess member needs and guide them toward appropriate care resources. This role involves face-to-face and telephonic assessments, coordinating with an interdisciplinary team to reduce care gaps and improve outcomes.

Travel to regions for in-person visits is expected. The position requires active Illinois licensure (LCSW/LMFT/LCPC), 2+ years of clinical behavioral health experience, and eligibility for Humana's TB screening and driver

Qualifications

• Active Illinois licensed LCSW, LMFT or LCPC (No provisional licenses).
• Must reside in Illinois or a border state.
• Minimum of 2 years of post-degree clinical experience in behavioral health setting.

Responsibilities

• Utilize high-quality evidence-based behavioral health services through personalized care coordination and collaboration with providers.
• Provide comprehensive, integrated support to members with behavioral health conditions (SMI, SED, SUD).
• Engage members in their communities through face-to-face visits when possible to build trust and coordinate care.
• Complete required assessments, including the Comprehensive Risk Assessment (HRA).
• Coordinate behavioral health and medical services, ensuring provider engagement and adherence to treatment plans.
