Job summary

Humana is seeking a Field Care Manager, Behavioral Health to join the Illinois Medicaid team. The role focuses on coordinating integrated behavioral health services, supporting members with SMI/SED/SUD, and guiding care planning with an interdisciplinary team.

You will travel up to 75% for face-to-face assessments, community visits, and collaborations with providers. Illinois license (LCSW/LMFT/LCPC) and 2+ years of post‑degree clinical experience are required; remote work with regular field

Qualifications

• Active Illinois license in LCSW, LMFT or LCPC.
• 2+ years of post‑degree clinical experience in a behavioral health setting.
• Ability to travel to region-based facilities and homes for face‑to‑face assessments.
• Ability to use a variety of electronic information applications/software programs including electronic medical records.
• Intermediate to advanced computer skills and experience with Microsoft Word, Outlook, and Excel.
• Valid driver’s license, car insurance, and reliable transportation.

Responsibilities

• Coordinate high-quality behavioral health services through care coordination and collaboration with medical and behavioral health providers.
• Provide integrated support to members with behavioral health conditions including SMI/SED/SUD.
• Engage members in their communities with face‑to‑face visits to build trust and facilitate care coordination.
• Coordinate behavioral health and medical services, ensuring provider engagement and adherence to treatment plans.
• Improve health literacy while addressing health-related social needs to improve outcomes.
• Serve as driver of the member's interdisciplinary care team and oversee ICT meetings and transitions.
