Job summary

Humana is seeking a Field Care Manager, Behavioral Health to join the IL Medicaid team. You will work with members face-to-face or by telecommunication, coordinating integrated behavioral health care and ensuring timely, high-quality services.

You will guide an ICT, lead care planning, and collaborate across internal and external partners to address barriers and improve outcomes. The role requires Illinois residency, an active LCSW/LMFT/LCPC license, and 2+ years of post-degree experience in

Qualifications

• Must reside in Illinois.
• Active Illinois license in LCSW, LMFT or LCPC (No supervisees or provisional licenses).
• 2+ years of post-degree clinical experience in behavioral health setting.
• Case management experience with complex SMI, SUD, SED population.
• Ability to travel to region-based facilities and homes for face-to-face assessments.
• Experience with electronic medical records and MS Office applications.

Responsibilities

• Utilize high-quality, evidence-based behavioral health services through personalized care coordination, crisis intervention, peer support, and collaboration with medical and behavioral health providers.
• Provide integrated support to members with behavioral health conditions across age groups including SMI/SED and SUD.
• Engage members in their communities with face-to-face visits to build trust and coordinate care.
• Coordinate behavioral health and medical services with provider engagement and treatment plan adherence.
• Improve health literacy and address social needs to positively impact outcomes.
• Serve as the driver of the interdisciplinary care team (ICT) and oversee care planning and transitions.
• Facilitate ICT meetings and ensure provider, Service Coordinators, and Care Management Extenders communication.
• Support transition of care efforts and collaborate with internal and external partners.
