Job summary

Humana Inc. is seeking a Field Care Manager, Behavioral Health 2 to assess needs and guide Medicaid members toward appropriate resources. This remote role involves face-to-face and telephonic assessments, coordination of care, and collaboration with providers to improve health outcomes.

Travel up to 25% is expected. The candidate must hold an active IL license (LCSW/LMFT/LCPC), reside in Illinois or a border state, and have at least 2 years of post-degree clinical experience in behavioral

Qualifications

• Active Illinois license in LCSW, LMFT or LCPC (no provisional licenses).
• Must reside in Illinois or a border state.
• Minimum of 2 years post-degree clinical experience in behavioral health.

Responsibilities

• Utilize evidence-based behavioral health services through care coordination, crisis intervention, and collaboration with providers.
• Provide integrated support to Medicaid members with behavioral health conditions across age groups.
• Engage members in their communities with face-to-face visits when possible to build trust.
• Complete assessments including Comprehensive Risk Assessment (HRA).
• Coordinate behavioral health and medical services and ensure adherence to treatment plans.
• Improve health literacy and address health-related social needs to impact outcomes.
• Serve as part of the interdisciplinary care team (ICT) and oversee care planning and transitions.
• Facilitate ICT meetings and ensure communication among providers and staff.
