Job summary

Humana is seeking a Field Care Manager, Behavioral Health for the Illinois Medicaid team. The role is remote with up to 75% travel to facilities and member homes.

You will coordinate care for children, adolescents, and adults with serious mental illness or substance use disorders, collaborating with providers and community organizations to deliver integrated, culturally competent care. Responsibilities include leading the ICT, coordinating services, and ensuring adherence to treatment plans,

Qualifications

• Must reside in Illinois.
• Active Illinois license in LCSW, LMFT or LCPC (no supervisees or provisional licenses).
• 2+ years of post-degree clinical experience in a behavioral health setting.
• Case management experience with complex SMI, SUD, SED populations.
• Ability to travel to region-based facilities and homes for face-to-face assessments.
• Ability to use a variety of electronic information applications/software programs including electronic medical records.
• Intermediate to advanced computer skills and experience with Microsoft Word, Outlook, and Excel.
• Valid driver’s license, car insurance, and reliable transportation.

Responsibilities

• Utilize high-quality, evidence-based behavioral health services through personalized care coordination and crisis intervention.
• Provide comprehensive, integrated support to members including children, adolescents, and adults with SMI/SED or SUD.
• Engage members in their communities with face-to-face visits when possible to build trust and coordinate care.
• Coordinate behavioral health and medical services and ensure adherence to treatment plans.
• Improve health literacy and address health-related social needs to improve outcomes.
• Lead the member’s interdisciplinary care team (ICT) and oversee care planning and transitions.
• Facilitate ICT meetings and ensure communication among providers and care staff.
• Support transition of care efforts proactively.
• Collaborate with internal departments and community organizations to link to services, respecting member preferences.
