Job summary

Humana is seeking a Field Care Manager, Behavioral Health 2 to assess and guide Medicaid members toward optimal wellness, coordinating with providers and families for integrated care.

Responsibilities include face-to-face and telephonic assessments, care planning, and coordinating services across disciplines. Travel up to 25% and remote work are required; strong regulatory and TB screening participation are part of the role.

Qualifications

• Active Illinois licensed LCSW, LMFT or LCPC (No provisional licenses)
• Must reside in Illinois or a border state
• Minimum of 2 years of post-degree clinical experience in behavioral health
• Case management experience with complex SMI, SUD, SED populations
• Ability to travel to region-based facilities and homes for face-to-face assessments

Responsibilities

• Provide integrated behavioral health care management through assessments and coordination
• Engage members in communities with face-to-face visits when possible
• Complete required assessments including Comprehensive Risk Assessment (HRA)
• Coordinate behavioral health and medical services with providers
• Support transitions of care and ICT meetings with care team
