Job summary

Humana is seeking a Field Care Manager, Behavioral Health to support the IL Medicaid program. You will coordinate integrated care, travel to facilities and homes, and engage members face-to-face to reduce gaps and improve outcomes.

The role requires an active Illinois BH license and 2+ years of post-degree experience, with up to 75% travel. Remote role with travel, pay range $65,000–$88,600 and a $5,000 hiring bonus program.

Qualifications

• Must reside in Illinois.
• Active Illinois license in LCSW, LMFT or LCPC (No supervisees or provisional licenses).
• 2+ years of post-degree clinical experience in behavioral health setting.
• Case management experience with complex SMI, SUD, SED populations.
• Ability to travel to region-based facilities and homes for face-to-face assessments.
• Experience with EMR/EHR systems and common Microsoft Office tools.

Responsibilities

• Utilize evidence-based behavioral health services through care coordination, crisis intervention, peer support, and collaboration with medical/behavioral health providers.
• Provide integrated support to members with BH conditions across age groups including SMI/SED and SUD.
• Engage members face-to-face in their communities to build trust and coordinate care.
• Coordinate behavioral health and medical services with provider engagement and adherence to plans.
• Improve health literacy and address health-related social needs to improve outcomes.
• Lead the interdisciplinary care team (ICT), oversee care planning, transitions, and service delivery.
