Job summary

Dahl Consulting is seeking a Behavioral Health Case Manager for a fully remote, contract (14 months) role in Wisconsin. The position focuses on telephonic case management for members with complex behavioral health needs, supporting navigation of healthcare resources and coordinated care to improve outcomes.

Responsibilities include comprehensive assessments, care plan development, cross-team collaboration, and ongoing progress monitoring.

Qualifications

• RN license in the state of residence (unrestricted)
• MSW with an unrestricted advanced practice Social Work license in the state of residence

Responsibilities

• Conduct comprehensive behavioral health assessments to identify member needs and barriers to care.
• Develop, implement, and monitor individualized care plans.
• Coordinate services across healthcare providers and community resources.
• Engage members telephonically to provide education and care coordination.
• Collaborate with families, caregivers, physicians, behavioral health providers, and interdisciplinary teams.
• Monitor care plan effectiveness and adjust interventions as needed.
