This organization serves diverse member populations through innovative, member-centered care management programs designed to improve health outcomes, enhance quality of life, and support efficient healthcare utilization.We are seeking a Behavioral Health Case Manager to provide comprehensive telephonic case management services for members with complex behavioral health needs. This fully remote position plays a critical role in helping individuals navigate the healthcare system, access appropriate resources, and achieve their care goals through evidence-based interventions and coordinated support.

Job DescriptionAs a Behavioral Health Case Manager, you will partner with members, families, caregivers, healthcare providers, and interdisciplinary teams to deliver holistic, individualized care. You will conduct comprehensive assessments, develop and implement care plans, coordinate services across the healthcare continuum, and monitor member progress to promote optimal outcomes.

Key Responsibilities:

• Conduct comprehensive behavioral health assessments to identify member needs, risks, and barriers to care.
• Develop, implement, and monitor individualized care plans based on clinical best practices and member goals.
• Coordinate services across healthcare providers, community resources, and support systems.
• Engage members telephonically to provide education, support, and care coordination.
• Collaborate with families, caregivers, physicians, behavioral health providers, and interdisciplinary teams.
• Assist members in navigating healthcare services and accessing appropriate resources.
• Support members with chronic mental health conditions and complex healthcare needs.
• Monitor care plan effectiveness and adjust interventions as appropriate.
• Promote improved health outcomes while supporting cost-effective utilization of healthcare resources.
• Maintain accurate documentation and manage multiple technology platforms to support daily case management activities.

QualificationsRequiredEducation & Licensure

One of the following:

• Associate or Bachelor's degree in Nursing with a current, unrestricted RN license in the state of residence; OR
• Master's degree in Social Work with a current, unrestricted advanced practice Social Work license in the state of residence

Experience

• 5+ years of clinical nursing or social work experience with a focus on behavioral health
• Experience working with vulnerable, high-risk, or complex populations in a clinical, home care, or telephonic setting
• Experience utilizing multiple computer systems, applications, and documentation tools
• Experience working with individuals from diverse age groups, cultural backgrounds, and socioeconomic populations

Knowledge, Skills & Abilities

• Strong understanding of behavioral health conditions and the healthcare continuum
• Excellent analytical and clinical decision-making abilities
• Strong verbal and written communication skills
• Ability to manage multiple complex cases simultaneously
• Excellent customer service and relationship-building skills
• Ability to work effectively in a dynamic, rapidly changing environment
• Strong resource coordination, problem-solving, and advocacy skills
• Ability to work independently in a remote environment while collaborating across teams

Preferred

• Direct case management experience
• Clinical licensure for Social Work professionals
• Certified Case Manager (CCM) certification
• Experience supporting members through telephonic case management programs
• Experience collaborating across multidisciplinary healthcare teams
• Knowledge of managed care, health plan operations, and care coordination programs
• Demonstrated success driving positive member outcomes through behavioral health interventions
