Job summary

Endeavors Inc. is seeking a Behavioral Health Case Manager to work within a multidisciplinary team, delivering case management to adults and families in-office and remotely.

You will conduct psychosocial assessments, develop recovery plans, connect clients with community resources, and document all services in an electronic health record. The role requires collaboration with partners, adherence to privacy and state guidelines, and a minimum of 60% direct client hours.

Qualifications

• Bachelor’s degree in a human services field and knowledge of mental health diagnoses.
• Valid Texas Driver’s License required.
• Experience in case management or crisis intervention preferred.

Responsibilities

• Collaboratively work with the Community Behavioral Health Services Department while participating in the multi-disciplinary team approach to client care.
• Provides case management to individuals experiencing housing instability and chronic mental health and/or substance use needs.
• Provides appropriate referrals as necessary.
• Prepares and maintains confidential case records.
• Consults with service providers and community partners on resource-related issues and community integration.
• Ensures confidentiality and controls access to sensitive information.
• Collaborates with clinical team to perform risk management and crisis intervention services as necessary.
• Develops network of trusted contacts within organizations in which to refer and coordinate care.
• Participates in multidisciplinary team meetings.
• Assists individuals/families to navigate service providers across multiple systems of care.
• Provides services via telehealth platform, as required.
• Perform other duties assigned by Community Behavioral Health Services
