Job summary

Blue Cross & Blue Shield of Rhode Island seeks a Behavioral Health Care Manager to direct and coordinate care for D-SNP members. You will complete health risk assessments, develop interdisciplinary care plans, and collaborate with nurses, social workers, dietitians, pharmacists, and care navigators to support integrated care delivery.

We require LICSW or LMHC licensure, 3–5 years in a medical/clinical or managed care setting, and CCM certification within three years.

Qualifications

• LICSW or LMHC license/certification required.
• Three to five years’ experience in medical/clinical environment or managed health plan.
• Must obtain CCM certification within three years of employment.

Responsibilities

• Manage members through the behavioral healthcare delivery system.
• Assess needs to identify appropriate interventions.
• Conduct health risk assessments and coordinate care plans.
• Facilitate communication among members, providers, and care team.
• Support self-management and SMART goals with health navigators.
• Promote member and provider satisfaction with benefits.
