Job summary

Horizon Blue Cross Blue Shield of New Jersey is seeking a clinician to perform behavioral health care management across the continuum of care from remote locations. The role involves assessing needs, coordinating services, and advocating for members with a focus on cost-effective, high-quality care.

Requires at least two years of behavioral health clinical experience, NJ licensure (LCSW/LMFT/LPC/RN), and proficiency with MS Office and intranet tools.

Qualifications

• High School Diploma or GED required; Bachelor’s degree preferred or relevant experience.
• Active NJ LCSW, LMFT, LPC or RN license is required.

Responsibilities

• Assesses behavioral health needs against guidelines to ensure appropriate level of care for inpatient stay.
• Evaluates necessity, appropriateness and efficiency of services provided.
• Coordinates and assists in implementing care plans for members.
• Monitors and coordinates services outside the network and negotiates fees as appropriate.
