Job summary

CarePoint Response in Philadelphia is seeking a Call Center Counselor to support individuals in treatment by coordinating transitions across the system of care. You will assess needs across mental health, medical, and social service domains and initiate referrals to internal and external providers to ensure timely access to services.

Qualifications

• High School Diploma or GED required.
• College degree in Human Services preferred.
• One year of work engagement or experience with individuals with behavioral health needs preferred.

Responsibilities

• Supports the delivery of services by participating in supervision and team meetings.
• Assess needs across mental health, medical, social, and other services.
• Initiates and submits referrals to medical and behavioral health care and community providers.
• Develops knowledge of care partners, service providers, and stakeholders.
• Collaborates with care teams to facilitate admissions and transitions.
