Qualifications

• Requires a Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing with 2–4 years of related experience.
• Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC) or RN per state contract requirements.

Responsibilities

• Evaluates member needs via phone or in-home visits to facilitate care plans.
• Develops ongoing care plans and identifies providers and community resources.
• Coordinates between member/family and care teams to ensure access to services.
• Monitors care plans and outcomes; suggests adjustments as needed.
• Educates members and families on procedures, referrals, and benefits.
