Job summary
Centene Corp. is seeking a Care Manager (BH) to develop and coordinate complex care plans for behavioral health and substance use needs, with foster care members in NW Kansas. Role supports remote work from home with up to 75% local travel and a Monday–Friday schedule.
The position requires a Master’s in Behavioral Health or Social Work or a Nursing degree with BH experience and 2–4 years of related work. Licensed BH professional or RN credential is required per state contract, plus a $5,000
Qualifications
- Requires a Master's degree in Behavioral Health or Social Work or a Nursing degree with BH experience.
- 2–4 years of related experience required.
- Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements.
Responsibilities
- Evaluate member needs via phone or in-home visits to tailor care plans.
- Develop and facilitate care plans for behavioral health and social determinants of health.
- Coordinate between members, families, and providers to ensure access to services.
- Monitor outcomes and adjust plans based on treatment responses.
- Provide education to members and families about procedures, treatments, and benefits.
