Qualifications

- 2–4 years of related experience in behavioral health care management.
- Licensed Behavioral Health Professional or RN per state requirements.
Responsibilities
- Evaluate member needs via phone or home visits to develop care plans.
- Coordinate with providers and community resources for access to care.
- Develop ongoing care plans for high acuity members and monitor outcomes.
- Educate members and families on procedures, options, and benefits.
- Ensure compliance with state, federal, and payer regulations.
