Qualifications

• Spanish-speaking bilingual preferred.
• Bachelor's or Master's degree in social work, psychology or related field is required or preferred.
• Ability to travel as needed and maintain reliable transportation.

Responsibilities

• Assess client needs and develop personalized care plans.
• Coordinate with services and supports to connect clients with resources.
• Lead in developing and implementing the consumer’s care plan and authorize services.
• Complete home visits and monitor services in alignment with program requirements.
