Qualifications

• 2+ years in healthcare coordination or related roles.
• Strong verbal and written communication with patients.
• Ability to multitask and manage follow-up tasks independently.
• Proficient with multiple communication channels (phone, email, chat).
• Detail-oriented and proficient in documenting outreach activity.
• Able to work remotely with a private workspace.
• Experience with productivity/reporting platforms.

Responsabilités

• Coordinate patient intake and scheduling, verify eligibility, and manage referrals.
• Engage patients to schedule appointments and complete pre-visit readiness tasks.
• Support provider panels, waitlist management, and outreach campaigns.
• Track outreach outcomes and escalate workflow barriers as needed.
• Collaborate with operations, providers, and cross-functional teams.
