Job summary

CenterWell Home Health, a Humana subsidiary, seeks an AVP, Clinical Product & Strategy to set strategic direction for clinical programs serving Medicare Advantage and Traditional Medicare populations. You will translate enterprise priorities into scalable home-based care models impacting 400,000+ patients across 37 states.

You will lead a ~20-person team partnering across Home Health, Pharmacy, Primary Care, and Humana to pilot and scale programs.

Qualifications

• Strategic, enterprise-oriented leader with strong clinical and value-based care acumen.
• Experience building, piloting, and scaling clinical programs with measurable outcomes.
• Ability to influence across matrixed teams including clinical, operations, finance, analytics, and payer partners.
• Strong command of Stars, utilization, medical cost, risk adjustment, documentation, and patient engagement dynamics.

Responsibilities

• Own MA value creation strategy roadmap for clinical programs.
• Oversee execution of strategic clinical programs and cross-functional coordination.
• Drive home health utilization and reduce avoidable costs.
• Improve quality and Stars performance with cross-team collaboration.
• Advance member engagement and longitudinal care beyond a single episode.
• Expand high-risk member services and integration across ecosystems.
