You will assess needs in homes, develop personalized care plans, and connect clients with essential services while ensuring compliance with state and program requirements. The position requires a Bachelor’s degree in social work, counseling, psychology, or related field, or RN/LSW licensure, plus experience with chronic conditions or mental illness.

Qualifications

• Bachelor’s degree in social work, counseling, psychology, or related field.
• RN or LSW credential or active license is preferred.
• Strong interpersonal and written communication skills with diverse populations.

Responsibilities

• Maintain knowledge of state and program requirements.
• Understand Medicaid, behavioral health systems, and community resources.
• Conduct in-home assessments to determine eligibility and safety.
• Document contacts and interventions per policy and timelines.
• Assist with community resource referrals as appropriate.
• Adhere to CareStar policies and procedures.
• Maintain patient confidentiality at all times.
• Follow Acceptable Use Policy for CareStar information systems.
