Qualifications

• Active Massachusetts Pharmacist license in good standing.
• Residency or 2+ years ambulatory care clinical experience.
• Fully remote role with collaboration across care teams.

Responsibilities

• Provide Comprehensive Medication Management (CMM) with care teams.
• Develop medication management action plans with measurable outcomes.
• Coordinate with Cityblock and external providers on pharmacy goals.
• Offer drug information, dosing, interactions, and adverse effects guidance.
• Educate staff via in-services and develop clinical tools for teams.
• Develop pathways/protocols towards collaborative practice agreements.
