Job summary

Cityblock Health is seeking a licensed Massachusetts pharmacist for a fully remote role. You will provide Comprehensive Medication Management in collaboration with hub-based and virtual clinical care teams and support services as needed.

You will help develop medication plans, monitor outcomes, and coordinate with Cityblock and external providers to align interventions with patient goals. This role also emphasizes education, pathways development, and integration into team-based care models with

Qualifications

Active, unrestricted Massachusetts pharmacist license.
Residency: PGY-1 Pharmacy Practice Residency and PGY-2 Ambulatory Care Residency, or 2+ years of ambulatory care clinical experience.
Experience with medication management in a team-based care model.

Responsibilities

Provide Comprehensive Medication Management (CMM) with hub-based and virtual care teams.
Develop and monitor medication management plans with measurable outcomes.
Coordinate with Cityblock and external providers to align interventions with goals.
Provide drug information and support for dosing, interactions, and adverse effects.
Educate staff and develop tools for medication management and reconciliation processes.
Support development of disease‑state clinical pathways and protocols.
