Cityblock Health is seeking an Ambulatory Care Pharmacist for a 100% remote role. Massachusetts license required and must be in good standing.

You will promote rational, safe, and cost-effective therapy, collaborate with care teams, and participate in medication management and clinical pathways. The expected salary range is $150,000-$162,000 and the company offers health, life, retirement, PTO and equity benefits.

Competenze

• Residency: PGY-1 and PGY-2 ambulatory care, or two years of ambulatory care clinical experience.

Mansioni

• Provide Comprehensive Medication Management (CMM) with care teams.
• Develop medication management plans with measurable outcomes and follow-up.
• Coordinate with internal and external providers to align medication goals.
