Job summary

Cityblock Health is hiring a licensed pharmacist for a fully remote US role. You must hold an active Massachusetts pharmacist license at hire and keep it in good standing.

You will work with hub-based and virtual clinical care teams to deliver Comprehensive Medication Management and develop actionable plans with measurable outcomes. Required qualifications include PGY-1 and PGY-2 residencies, or at least 2 years of ambulatory care clinical experience.

Qualifications

• Residency in pharmacy or equivalent ambulatory care experience.
• Active Massachusetts pharmacist license required at hire and ongoing.
• Experience with medication management in ambulatory care settings.

Responsibilities

• Provide Comprehensive Medication Management (CMM) with hub-based and virtual teams.
• Develop medication management action plans with measurable outcomes.
• Collaborate with Cityblock and external providers to align interventions with goals.
• Offer drug information, dosing and interaction guidance to clinical staff.
• Educate staff on medication management practices and tools such as Commons.
• Help develop clinical pathways and move towards CPA with Cityblock physicians.
