Qualifikationen

• Completion of PGY-1 Pharmacy Practice Residency and a PGY-2 Ambulatory Care Pharmacy Residency
• Alternative: minimum of 2 years of experience as a clinical pharmacist in ambulatory care setting

Aufgaben

• Provide Comprehensive Medication Management (CMM) with hub-based and virtual teams
• Develop medication management action plans with measurable outcomes and timelines
• Collaborate with Cityblock and external providers to align interventions with medication goals
• Provide drug information and dosing support to clinical team
• Educate staff on medication management, tools, and post‑discharge reconciliation processes
• Help develop disease-state clinical pathways and pursue collaborative practice agreements
