Job summary

Cityblock Health is seeking an experienced Ambulatory Care Clinical Pharmacist for a fully remote role. Candidates must hold an active Massachusetts pharmacist license and maintain it in good standing.

The role centers on delivering Comprehensive Medication Management and collaborating with hub-based teams to optimize care. Qualified applicants should have PGY-1 and PGY-2 residencies or at least 2 years of ambulatory care experience, with a focus on safe, cost-effective therapy and population

Qualifications

• At least 2 years of experience as a clinical pharmacist in an ambulatory care setting.
• Residency training in PGY-1 and PGY-2 ambulatory care is required.

Responsibilities

• Provide Comprehensive Medication Management (CMM) with clinical care teams.
• Develop and refine medication management plans and pathways.
• Educate staff and participate in training and coaching of team members.
• Assist in post-discharge medication reconciliation and chronic disease management.
