Job summary

Cityblock Health is seeking a licensed pharmacist for a fully remote role in the United States. An active Massachusetts pharmacist license is required at hire and must be maintained.

This position will provide Comprehensive Medication Management in collaboration with hub-based and virtual care teams, developing medication action plans with measurable outcomes and ensuring alignment with patient goals. Candidates may have PGY1 and PGY2 residencies or at least 2 years of ambulatory clinical

Qualifications

• Active Massachusetts pharmacist license required
• Must hold an active, unrestricted pharmacist license from the Massachusetts Board of Registration in Pharmacy at time of hire
• Residency and/or ambulatory care clinical experience required as stated

Responsibilities

• Provide Comprehensive Medication Management (CMM) with hub-based and virtual teams.
• Develop medication management action plans with measurable outcomes.
• Coordinate with Cityblock and external providers to align medication-related goals.
• Provide drug information and dosing guidance to clinical teams and members.
• Lead staff education on medication management and contribute to training materials.
• Help develop clinical pathways and work toward CPA-based practice with physicians.
