Job summary

The ALS Association is the world’s leading ALS organization, delivering research funding, patient support, and advocacy. We are seeking aremote, part-time Care Services Coordinator in Alaska to provide comprehensive services to people with ALS, their families, and caregivers.

Reporting to the Care Services Director, you will register clients, assess needs, coordinate plans of care, connect clients with equipment and services, and document activities in Salesforce.

Qualifications

• Bachelor’s degree in social work, nursing, public health, or related area. Occupational therapist, physical therapist, speech therapist or direct experience with ALS may be considered.
• Associate's Degree may be considered if combined with 3+ years of experience in a similar role.
• Ability to work independently with little supervision; must be a self-starter.
• Knowledge of Medicare, Medicaid, patient assessments skills, planning and case management preferred.
• Commitment to organization and highly efficient and conscientious about thorough follow-up.
• Skilled communicator, with superior interpersonal, verbal, and written skills.
• Able to handle confidential information and maintain high confidentiality.
• Excellent organizational and writing skills as well as diligent attention to detail.
• Ability to drive locally and travel nationally on Association business is required.

Responsibilities

• Respond to inquiries for information on ALS and community resources.
• Assist with registration with The ALS Association and ALS National Registry.
• Collect patient, medical, and demographic information to establish a care plan.
• Assess physical and psychosocial needs related to ALS.
• Conduct care consultations and needs assessments in multiple settings.
• Advocate for equipment, services, and care to improve quality of life.
• Maintain quarterly contact with clients and caregivers.
• Document engagements in Salesforce.
• Educate the public and professionals about ALS and available services.
• Educate clients about local resources and available options.
• Coordinate support groups and arrange speakers as needed.
• Serve as liaison between the clinic and The ALS Association.
