SupportFinity™ is seeking a Care Services Manager to oversee care services for individuals with ALS. You will develop community relationships, manage service delivery, and facilitate educational resources for patients and caregivers.

This remote position requires a Bachelor's degree in social work or a related field, with a preference for a Master's degree. You should have at least 3-5 years of experience in care management and strong communication skills.

The salary range is $50,649 - $79,347 annually, and benefits include healthcare coverage, 401(k) plan, and paid time off.

Qualifications

• 3-5 years of professional experience in direct care and case management.
• Ability to work independently with little supervision.
• Proficiency in using a variety of communication forms.

Responsibilities
