Job summary

The ALS Association is seeking a Care Services Manager to develop professional and community relationships that enable high-quality, consistent care for people living with ALS and their families. You will oversee care services programs, supervise staff in the territory, and facilitate education, benefit, and clinical resource navigation support.

Qualifications

  • Bachelor’s degree or related experience in social work, case management, health administration, or nursing.
  • Minimum of four years of related professional experience and/or training.
  • Strong problem-solving, budgeting, and planning skills.
  • Knowledge of ALS is a plus, but not required.
  • Experience with progressive neurodegenerative disease.
  • Knowledge of Medicare/Medicaid, patient assessments and case management preferred.

Responsibilities

  • Conduct specialized care consultations and needs assessments for people living with ALS, caregivers, and families.
  • Manage a portfolio of clients and supervise field staff in the territory.
  • Provide information on programs, services, resources and educational materials.
  • Coordinate referrals to healthcare providers and community agencies.
  • Evaluate and ensure effectiveness of support groups.
  • Collaborate with the CS Director and Managing Director on program outcomes and evaluation.
  • Ensure quarterly contact with those living with ALS and caregivers.
  • Document all engagements per Care Services Standards.
  • Educate and inform the public and professionals about ALS and the Association’s services.
  • Serve on the ALS Clinic multidisciplinary team and liaison with the clinic and the Association.
  • Provide in-service educational programs to ALSA staff and healthcare professionals.
  • Participate in continuing education and mentoring for new staff.
