Job summary

The ALS Association is seeking a Care Services Manager to lead care services programs and build professional and community relationships across an assigned territory. You will ensure high-quality, consistent services for individuals with ALS and their families, and supervise care services staff as needed.

Qualifications

• Bachelor's degree in Social Work, Nursing, Human Services, Case Management, or a closely related field required.
• Master's degree in social work preferred.
• Minimum of 4 years of related professional experience and/or training.
• Knowledge of Medicare, Medicaid and patient assessments preferred.
• Experience with progressive neurodegenerative disease is a plus.
• Ability to manage programs, supervise staff and coordinate across teams.

Responsibilities

• Conducts specialized care consultations and needs assessments for people living with ALS, caregivers, and families.
• Manage a portfolio of clients and supervise field staff as needed.
• Provide information on programs, services, resources and educational materials.
• Coordinate referrals to healthcare providers and community agencies.
• Evaluate and support the effectiveness of ALS support groups.
• Plan implementation of program outcomes and evaluation in collaboration with leadership.
• Ensure quarterly contact/engagement with clients and caregivers.
• Educate the public and professionals about ALS and the Association's services.
• Serve as liaison between clinics and The ALS Association; maintain clinic relationships.
• Provide in-service educational programs to staff and healthcare professionals.
• Participate in continuing education and mentoring opportunities.
