Job summary

The ALS Association in Boise, Idaho is seeking a Care Services Coordinator to provide comprehensive support, education, and advocacy to people affected by ALS and their families. You will respond to inquiries, connect clients to resources, coordinate care plans, and document engagements in Salesforce, while working with the Care Services team.

Ideal candidates have a relevant degree, strong communication skills, ability to work independently, and willingness to travel nationally as needed.

Qualifications

  • Bachelor’s degree in social work, nursing, public health, or related area; OT/PT/SLT or ALS experience may be considered.
  • Associate’s Degree may be considered with 3+ years of relevant experience.
  • Ability to work independently with little supervision; must be a self-starter.
  • Knowledge of Medicare/Medicaid and patient assessments and case management preferred.
  • Excellent writing and communication skills.

Responsibilities

  • Respond to inquiries for information on ALS and community resources.
  • Assist persons living with ALS to register with The ALS Association and the ALS National Registry.
  • Coordinate plan of care by receiving referrals and gathering necessary information.
  • Assess physical and psychosocial needs based on ALS knowledge.
  • Conduct care consultations in various settings including in person and virtual.
  • Advocate to obtain equipment and services to improve quality of life.
  • Maintain quarterly contact with clients and caregivers.
  • Connect clients to programs and services and document engagements in Salesforce.
  • Educate the public and professionals about ALS services and resources.
  • Facilitate support groups and schedule speakers.
  • Serve as liaison between the clinic and The ALS Association.
  • Participate in program measurement and cultivate relationships with allied professionals.
  • Provide in-service education to home health, DME providers, and SNFs.
  • Participate in continuing education and related activities.
