Job summary

The ALS Association is seeking a Care Services Manager to develop professional and community relationships and to manage care services programs for individuals with ALS and their families. The role includes education, resource navigation, and coordination with healthcare providers within an assigned territory.

Responsibilities include conducting needs assessments, supervising care services staff, and ensuring high-quality, consistent services.

Qualifications

• Bachelor’s degree or related experience in social work, case management, health administration, allied and public health, nursing, or a related area required.

Responsibilities

• Conducts specialized care consultations and needs assessments for people living with ALS, caregivers, and families.
• Actively manage a portfolio of clients to ensure programs meet the needs of people living with ALS and caregivers.
• Coordinate referrals to healthcare providers and community agencies.
• Evaluate and ensure the effectiveness of support groups within the ALS community.
• Plan program outcomes and evaluation processes with CS Director and Managing Director.
• Ensure quarterly contact/engagement with people living with ALS and/or caregivers.
• Document engagements per Care Services Standards and educate the public about ALS.
• Serve as liaison between clinics and The ALS Association; maintain clinic relationships.
