Job summary

Affinity Hospice is seeking a Centralized Triage Registered Nurse to provide compassionate, telephone-based nursing care to hospice patients and families after hours, weekends, and holidays. The role is remote, covering service areas across multiple states, and focuses on symptom management, guidance, and coordination with the interdisciplinary team.

Qualifications

• Associate’s degree in nursing is required; RN licensure active.
• Multistate (compact) license required or willingness to obtain.
• Current CPR certification is required.
• Valid driver’s license and reliable transportation for potential home visits.

Responsibilities

• Serve as primary point of contact for hospice patients and families during after-hours, weekends, and holidays.
• Triage calls, assess patient needs, provide clinical guidance by phone.
• Resolve non-clinical requests or route them to appropriate party.
• Collaborate with interdisciplinary team to ensure symptom management and continuity of care.
• Make clinical decisions based on patient status and plans of care.
• Provide symptom management instructions for pain, breathing difficulty, and other concerns.
• Offer emotional support to families and caregivers.
• Educate patients and families on end-of-life care and comfort measures.
• Coordinate after-hours visits by hospice field nurses to ensure patient comfort.
• Document triage interactions and nursing assessments in EMR promptly.
• Provide detailed reports to nursing team, physicians, and case managers.
