Job summary

Moments Hospice seeks a Triage RN to join the interdisciplinary team in an after-hours, fully remote role. You will assess needs, provide guidance, and coordinate care using the nursing process and Telecare protocols.

Ideal candidates are experienced hospice RNs who work independently while delivering compassionate, patient-centered care, with strong communication and timely documentation in the EMR.

Qualifications

• Active RN license (no restrictions).
• Hospice experience required.
• Home Care Home Base (HCHB) experience preferred.

Responsibilities

• Provide after-hours triage and clinical assessment via phone.
• Support patients and families with symptom management and end-of-life care needs.
• Coordinate care with physicians, facilities, and internal teams.
• Dispatch visits and coordinate services with vendors.
• Deliver clear, compassionate education and support to families.
• Document all interactions and care plans in the EMR.
• Review team documentation for accuracy and compliance.
• Partner with Telecare staff to ensure positive patient outcomes.
