Job summary

Interim HealthCare of the Twin Cities is seeking a licensed RN in MN for after-hours on-call triage and in-home visits. You will respond to patient calls, assess needs, and provide care coordination while ensuring safety and comfort in the patient’s home.

This full-time role involves scheduling on a 7 on/7 off cycle, with after-hours hours and weekends. We offer competitive salary, mileage reimbursement, PTO, 401k, health insurance, and ongoing professional support.

Qualifications

• Must be licensed to practice as a RN in MN, active and in good standing.
• Minimum of 12 months RN experience within the last 5 years or current clinical knowledge (last 12 months).
• One year of hospice or home care experience preferred.
• Valid driver’s license, reliable transportation, and auto insurance proof.
• CPR certification required.

Responsibilities

• Triage calls after hours and determine care needs for patients.
• Perform in-home visits when necessary within the service area.
• Coordinate with care teams to ensure continuity of care.
