Job summary

Optimal Care in Michigan is seeking a Triage Coordinator to manage after-hours calls, orders, and communications with on-call providers, patients, facilities, and families for Physician Services.

The role requires initiative, creative problem resolution, time management, and excellent verbal and written communication to support a geriatric population, while maintaining confidentiality and high-quality care.

Qualifications

• Strong verbal and written communication skills
• Ability to operate computer equipment and office machines (fax/photocopy)
• Knowledge of medical terminology
• Experience in medical field or medical office preferred
• Ability to work under stress and in after-hours situations
• Excellent time management and problem-solving abilities

Responsibilities

• Communicates significant findings or changes to primary care providers and other personnel involved in patient care
• Performs well under stress and in emergency situations
• Assumes primary responsibility for after-hours planning, coordinating, and evaluating the care plan for patients, families, and facilities
• Develops positive working relationships with others and assists as needed
• Maintains confidentiality of patient and employee information
• Answers after-hours calls and responds to concerns or changes in patient condition
• Provides support and direction to agency staff, healthcare professionals, patients, and families related to health concerns
