Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team in Michigan. The role focuses on clinical triage, care coordination, and patient education to improve outcomes and the patient experience.

Qualifications

• Valid, unrestricted RN license in Michigan.
• BSN strongly preferred or graduate of an accredited RN program.
• Minimum 3 years of ambulatory care or related clinical experience.

Responsibilities

• Triage and coordinate patient care by phone and in-person visits.
• Provide clinical guidance, education, and care coordination.
• Support transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, and behavioral health specialists.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Assist with hiring, staff motivation, and engagement where applicable as a nurse leader.
• Address at-risk patient needs to improve health outcomes.
• Navigate patients through care pathways and community resources.
