Qualifications

• Unrestricted LMSW license in Michigan.
• 4+ years of experience working in community health settings.

• 1+ years of experience with MS Word, Excel, and Outlook.
• 1+ years of experience with Michigan Medicaid managed care.
• Access to reliable transportation and a valid US driver’s license.

Responsibilities

• Assess, plan, and implement individualized care strategies.
• Manage the care plan as the single point of contact.
• Communicate health information to stakeholders for coordinated care.
• Advocate for patients and families to reflect needs and choices.
• Provide telephonic care management to caseloads including specialized populations.
• Support outreach to recently discharged members to prevent readmissions.
• Present and participate in interdisciplinary rounds.
• Collaborate with Medical Directors on member needs and complex issues.
• Support Quality and Population Health initiatives as requested.
• Participate in work groups to improve processes and workflows.
