Job summary

UPMC seeks a Manager, Home First Center Care Management to provide operational leadership for Telephonic Care Managers supporting patients during critical transitions in care. You will lead a 24/7 operation, coordinate staffing and workflows, and ensure high-quality, patient-centered care coordination across the continuum.

In this role within the Integrated Delivery & Finance System (IDFS), you will partner with hospital, ambulatory, post-acute, and payer teams to improve patient outcomes,

Qualifications

• Requires RN with active licensure (or equivalent clinical leadership experience).
• Experience leading care management, discharge planning, or transitions of care.
• Experience supervising or managing teams.

Responsibilities

• Lead daily operations of the Home First Center and ensure 24/7 coverage.
• Develop staffing, scheduling, and workflow strategies for operational demands.
• Model and promote best practices in care management and transitions of care.
• Partner with hospital, ambulatory, post-acute, community, and IDFS stakeholders.
