Job summary

UPMC's 24/7 Home First Center (HFC) is seeking a Manager to lead Telephonic Care Managers delivering care coordination across transitions in care. You will oversee daily operations, staffing, and 24/7 coverage to ensure uninterrupted patient services.

You will partner with hospital, ambulatory, post-acute, community and IDFS colleagues to advance care coordination strategies, improve patient experiences, and support organizational goals in a growing program focused on safer transitions of care.

Qualifications

• RN with active licensure required.
• Bachelor's Degree in Nursing required; Master's Degree preferred.
• Five (5) years of progressive care management, utilization management, discharge planning, transitional care, or related clinical leadership experience.
• Prior supervisory or management experience preferred.
• CCM, ACM, or equivalent certification preferred.

Responsibilities

• Lead daily operations of the Home First Center and ensure high-quality care coordination with 24/7 coverage across shifts.
• Develop staffing, scheduling, and workflow strategies to support operational demands and team effectiveness.
• Monitor performance, productivity, and outcomes; identify opportunities to improve efficiency and patient experience.
• Provide leadership, coaching, and clinical guidance to Telephonic Care Managers, ensuring consistent care management practices.
