Job summary

Walnut Street Center in Bedford, MA is seeking Relief/Per Diem Case Workers to empower adults with intellectual disabilities in home and community settings. You will support daily living and help individuals pursue meaningful life goals.

Responsibilities include developing and documenting goals, providing therapeutic support, transporting clients to appointments, and assisting with daily living activities and money management.

Qualifications

• High School Diploma or equivalent is required.
• Valid MA driver's license and willingness to drive a personal vehicle.
• Ability to pass driving and background checks.
• Ability to communicate effectively in English.
• Willingness to complete training within 90 days.

Responsibilities

• Assist in maintaining a safe and livable environment.
• Help develop, implement, and document each individual's goals and progress.
• Provide therapeutic support and contribute to Behavior Support Plans when appropriate.
• Transport individuals to appointments and activities.
• Assist with Activities of Daily Living and personal care tasks.
• Support money management and benefits as needed.

Per Diem/Relief: Days (10a-6p) and Weekends (9a-3p). 20 Hours of Availability Preferred

Who we are looking for:

Seeking per diem case workers to help empower adults with intellectual disabilities to make more meaningful life choices by working directly with those individuals in our Independent Living Supports (ILS) Department

Work Settings:

Individual's Homes/In the Community

The Job Responsibilities:

• Assisting with ensuring a livable environment
• Participating in the development, implementation, evaluation, and documentation of each individual's goals
• Providing therapeutic support and assisting in Behavior Support Plans, if appropriate
• Transporting individuals to and from appointments
• Assisting individuals with Activities of Daily Living such as feeding, dressing, toileting, and other aspects of personal care, and nutritional needs
• Money management, such as support with benefits
• Willingness to obtain training & certification within 90 days of hire - We will train!

Job Requirements:

• High School Diploma or equivalent
• Valid MA driver’s license and willingness to drive personal vehicle
• Ability to pass a driving and background check
• Ability to communicate effectively in English
• Ability to complete training both online and in-person

Preferred:

• Previous human services or long-term care experience is strongly preferred
• CNA/HHA experience preferred
• Associate’s or bachelor’s degree in a related field preferred
