Position Overview
- Developing and implementing Individual Service Plans (ISP).
- Supporting community inclusion and independent living skills.
- Providing care coordination, direct care, and supervision as needed—including personal care and daily living activities.

PRIMARY DUTIES:
Coordinate care and provide support in:
- Education and social skills
- Meal planning and grocery shopping
- Personal care and hygiene
- Clothing care and travel training
- Money management and housing
- Community participation and religious practices (if desired)
Transport individuals to medical appointments and:
- Follow up on medical care plans
- Educate individuals, families, and staff on self-directed care
Support healthy, culturally appropriate meal planning and purchasing
Provide transportation and skill-building in grocery shopping

Qualifications:
- Bachelor's degree (or currently enrolled) in Human Services or a related field
- Equivalent experience may be considered in some cases
- Preferred: Experience working with individuals with developmental disabilities
- Familiarity with behavior modification techniques
- Strong oral and written communication skills
- Valid Massachusetts Driver's License and reliable personal transportation
- Proficient in Microsoft Office and general computer use
LANGUAGE SKILLS:
- Fluent in English with the ability to read and interpret documents.
- Ability to communicate effectively with clients, colleagues, families, and community members.
- Strong written and verbal communication skills for reports, correspondence, and presentations.
PHYSICAL DEMANDS:
- Employee must have significant endurance to sustain the physical demands of the role.
- Regular activities include sitting, standing, walking, stooping, kneeling, or crouching.
- Frequent use of hands/fingers to handle objects and tools.
- Ability to generate force to maneuver up to 100 pounds (assistance provided when possible).
- Must be able to lift up to 30 pounds from floor to waist and 10-15 pounds from waist to overhead.
REASONING ABILITY:
Ability to follow detailed written and oral instructions.
