Job summary

Open Sky Community Services seeks a Relief Counselor to support individuals with mental health challenges, helping them live independently and pursue meaningful lives. This role is part of a relief team offering flexible scheduling in Central Massachusetts.

You will provide direct support, develop person-centered plans, assist with transportation and community resources, and help individuals build skills and relationships. Documentation duties are included.

Qualifications

• High School Diploma, GED or equivalent is required.
• Valid Driver’s License and acceptable driving record are required.
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Assist in developing and implementing individualized, person-centered support plans.
• Assist individuals in accessing community resources and/or provide transportation as needed.
• Implement activities that promote skill development, community engagement, personal growth, and meaningful relationships.
• Review and complete all required documentation.
